I vow to have one in mine someday. but the brokers or rebuilders usually get the KR edition straight from the insurance adjusters. good luck finding one that ain't gutted already.

Is the black hole on the interior of the B pillar a slot for a shoulder harness bolt?

Yes, there's a threaded hole, usually covered with a rubber plug, for a shoulder harness. It was actually a factory option for these trucks, yet very very few ever installed.
